Grant of rights By completing and submitting this form, I ("Participant") grant Last Crush LLC ("Company") a perpetual, worldwide, royalty-free, non-exclusive license to use, reproduce, publish, distribute, and display any photographs, videos, or other media featuring my likeness that were captured in connection with my participation in Last Crush-related activities, events, or content creation sessions.

Permitted uses This license covers use across all media formats and channels, including but not limited to: the Company's website (lastcrush.co), email marketing, organic social media posts on Instagram, TikTok, Pinterest, Facebook, and YouTube, product photography, lookbooks, press materials, and any other promotional or marketing purposes reasonably related to the Last Crush brand.

Paid advertising (separate consent) If I check the paid advertising consent box in the form below, I additionally grant Last Crush LLC the right to use my likeness in paid digital advertising, including promoted posts, Instagram Ads, TikTok Ads, and Google Display Ads. This is a separate and optional consent.

Compensation + approval I understand that I will not receive monetary compensation for this usage. I waive any right to inspect or approve the finished content before publication. I confirm that I am 18 years of age or older, or that a parent or legal guardian has provided consent on my behalf.

Governing law This release is governed by the laws of the State of Arizona, United States. A digital signature combined with a timestamped IP address record shall constitute a valid and binding signature for the purposes of this agreement.
